Periodontal Disease – Healthy gums may lead to healthy lungs

Maintaining periodontal health may contribute to a healthy respiratory system, according to research published in the Journal of Periodontology. A new study suggests that periodontal disease may increase the risk for respiratory infections, such as chronic obstructive pulmonary disease (COPD) and pneumonia. These infections, which are caused when bacteria from the upper throat are inhaled into the lower respiratory tract, can be severely debilitating and are one of the leading causes of death in North America.

The study included 200 participants between the ages of 20 and 60 with at least 20 natural teeth. Half of the participants were hospitalized patients with a respiratory disease such as pneumonia, COPD, or acute bronchitis, and the other half were healthy control subjects with no history of respiratory disease. Each participant underwent a comprehensive oral evaluation to measure periodontal health status.

 

The study found that patients with respiratory diseases had worse periodontal health than the control group, suggesting a relationship between respiratory disease and periodontal disease. Researchers suspect that the presence of oral pathogens associated with periodontal disease may increase a patient’s risk of developing or exacerbating respiratory disease. However, the study authors note that additional studies are needed to more conclusively understand this link.

“Pulmonary diseases can be severely disabling and debilitating,” says Donald S. Clem, DDS, President of the American Academy of Periodontology. “By working with your dentist, you may actually be able to prevent or diminish the progression of harmful diseases such as pneumonia or COPD. Gingivitis – types of gum disease

Periodontal (gum) diseases, including gingivitis and periodontitis, are serious infections that, left untreated, can lead to tooth loss. The word periodontal literally means “around the tooth.” Periodontal disease is a chronic bacterial infection that affects the gums and bone supporting the teeth. Periodontal disease can affect one tooth or many teeth. It begins when the bacteria in plaque (the sticky, colorless film that constantly forms on your teeth) causes the gums to become inflamed.

Gingivitis is the mildest form of periodontal disease. It causes the gums to become red, swollen, and bleed easily. There is usually little or no discomfort at this stage. Gingivitis is often caused by inadequate oral hygiene. Gingivitis is reversible with professional treatment and good oral home care.

Untreated gingivitis can advance to periodontitis. With time, plaque can spread and grow below the gum line. Toxins produced by the bacteria in plaque irritate the gums. The toxins stimulate a chronic inflammatory response in which the body in essence turns on itself, and the tissues and bone that support the teeth are broken down and destroyed. Gums separate from the teeth, forming pockets (spaces between the teeth and gums) that become infected. As the disease progresses, the pockets deepen and more gum tissue and bone are destroyed. Often, this destructive process has very mild symptoms. Eventually, teeth can become loose and may have to be removed.

There are many forms of periodontitis. The most common ones include the following.

  • Aggressive periodontitis occurs in patients who are otherwise clinically healthy. Common features include rapid attachment loss and bone destruction and familial aggregation.
  • Chronic periodontitis results in inflammation within the supporting tissues of the teeth, progressive attachment and bone loss. This is the most frequently occurring form of periodontitis and is characterized by pocket formation and/or recession of the gingiva. It is prevalent in adults, but can occur at any age. Progression of attachment loss usually occurs slowly, but periods of rapid progression can occur.
  • Periodontitis as a manifestation of systemic diseases often begins at a young age. Systemic conditions such as heart disease, respiratory disease, and diabetes are associated with this form of periodontitis.
  • Necrotizing periodontal disease is an infection characterized by necrosis of gingival tissues, periodontal ligament and alveolar bone. These lesions are most commonly observed in individuals with systemic conditions such as HIV infection, malnutrition and immunosuppression.

What is Periodontal Disease?

Periodontal disease (also called periodontitis or gum disease) is a bacterial infection that destroys the supporting structures that hold your teeth in place. Your gums are the first line of defense against bacterial infection, but once the infection has broken through them it will begin to attack the ligaments and bone that hold the teeth in the mouth. It is in this stage of periodontitis that loss of bone due to bacterial erosion can occur, which will then cause the pocket (space between the gum and tooth) to grow deeper and wider. Left untreated, this disease can lead to tooth loss.

What are the signs of periodontal disease?

Periodontitis is often a silent disease as those who suffer from it rarely experience pain, and may not even be aware that there is a problem. The early stages of gum disease are seldom painful, and carry with them very few warning signs. Some of the early warnings to look for are slight bleeding while brushing, softening of the gums, and discoloration of the gums (darker red/irritated in appearance rather than a light, healthy pink). As the disease progresses, more noticeable signs may become more predominant. Some examples of these more serious indicators are:

 

• Swollen gums • Sores in the mouth
• Inflamed gums • Bright red or purple gums
• Receding gums that make your teeth look longer • Trench mouth (thrush)
• Spaces developing between teeth • Loose teeth
• Persistent bad taste • Pus between the teeth and gum line (pyorrhea)
• Chronic bad breath (halitosis) • Change in the way teeth fit together when you bite
• Tender gums when touched

 

If you have any of the above signs, you could be significantly farther down the path of serious periodontitis than you think, and close to the potential of loss of your teeth.

Periodontal disease treatment involves both non-surgical and surgical options.  Usually, periodontal disease treatment is done non-surgically first and if the periodontal disease cannot be controlled or managed properly, then periodontal disease treatment involving surgery is needed.  Captial Smiledocs Dental, with offices both in Ottawa and Stittsville, offers both non-surgical and surgical periodontal disease treatment.

Treatment is done with scaling and root planing (a careful cleaning of the root surfaces to remove plaque and calculus [tartar] from deep periodontal pockets and to smooth the tooth root to remove bacterial toxins).

In most cases, after scaling and root planing, many patients do not require any further  surgical therapy. However, the majority of patients will require ongoing maintenance therapy to sustain health. Non-surgical therapy does have its limitations, however, and when it does not achieve periodontal health, surgery may be indicated to restore periodontal anatomy damaged by periodontal diseases and to facilitate oral hygiene practices.

Wondering If Your Brushing Techniques Is Good?

Proper brushing takes at least two minutes — that’s right, 120 seconds! Most adults do not come close to brushing that long. To get a feel for the time involved, try using a stopwatch. To properly brush your teeth, use short, gentle strokes, paying extra attention to the gumline, hard-to-reach back teeth and areas around fillings, crowns or other restoration. Concentrate on thoroughlycleaning each section as follows:

  • Clean the outer surfaces of your upper teeth, then your lower teeth
  • Clean the inner surfaces of your upper teeth, then your lower teeth
  • Clean the chewing surfaces
  • For fresher breath, be sure to brush your tongue, too
  • Avoid hard scrubbing
  • Hold your toothbrush at a 45-degree angle and gently brush in short strokes from where the tooth and gum meet to the top of the tooth
  • Clean the pits and crevices on the chewing surface of your teeth with short sweeping strokes
  • Proper brushing can help prevent gum recession and cavities

What Type of Toothbrush Should I Use?

A soft-bristled brush is best for removing plaque and debris from your teeth. Dental floss is a thin thread that is either waxed or unwaxed. It helps removes food particles from between the teeth and under the gumline. Flossing helps you clean 35% more tooth surface than you would by just brushing your teeth. It’s recommended that you floss daily.

When your child eats or drinks sugars, the germs (bacteria) in your child’s mouth mix with the sugars to make a mild acid. This acid attacks the hard outer layer of teeth (also called enamel). It can make holes (or cavities) in the teeth.

The damage that sugars do depends on how much sugar goes into the mouth and how long it stays in the mouth.

Any kind of sugar will mix with germs in the mouth. Natural sugars can have the same effect on teeth as white (or refined) sugar out of the bag! Many healthy foods contain natural sugars. Milk contains natural sugar.

If you put your child to bed with a bottle of milk, the milk stays in the mouth for a long time. This may cause cavities. Unsweetened fruit juice may have no added sugar, but fruit juice has natural sugars in it. If your child is always sipping juice between meals, the teeth are being coated in sugars over and over again.

Water is the best drink to have between meals. Starchy foods, like teething biscuits, break down to make sugars. If these kinds of food stay in your child’s mouth long enough, they will make the acid that can cause cavities. Your job is to clean your child’s teeth, not to stop your child from having milk, juice, bread or noodles. Your child needs these foods to stay healthy.

Read the labels of the packaged food you buy. By law, everything ingredient in packaged food is listed by weight. So if a sugar is listed first, you know that there is more sugar than anything else.

These are sugars you can look for on labels: corn sweeteners; corn syrup; dextrose; fructose; glucose; honey; maple syrup; molasses and sucrose.

Also, check to see if liquid medicines (such as cough syrup) have sugars. Ask your doctor or pharmacist to give you medicines that are sugar-free.

Growing children need and like snacks. Here are some smart ways to give snacks:

Limit the number of times a day your child eats or drinks sugars. If your child sips juice or pop while playing, he or she will have sugars in the mouth over and over again. Water is the best drink to have between meals.

Do not give your child sugar-rich foods that stay in the mouth for a long time like gum with sugar in it, suckers (or lollipops) and other hard candy. Stay away from soft, sticky sweets that get stuck in the mouth such as toffee, raisins and rolled-up fruit snacks or fruit leather.

Keep good snacks handy where your child can get them. Have carrot sticks or cheese cubes on the bottom shelf of the fridge. Children like small things like small boxes of cereal, small fruits and vegetables, and small packs of nuts or seeds (provided they are safe for your child). Keep them in a low cupboard.

To keep your child from asking for sweets, do not buy them. If they are not in the house, you can’t give them out. If you do serve sweets, limit them to meals. When your child is eating a meal, there is more saliva in the mouth. This helps to wash away the sugars.

Once your child has teeth, he is susceptible to tooth decay. Mother’s milk, formula, cow’s milk and fruit juice all contain sugars.

Babies may get early childhood tooth decay from going to bed with a bottle of milk, formula or juice. Unrestricted at-will breast-feeding at night may increase the risk of tooth decay, although the majority of breast-fed children do not experience this early childhood disease.

It can happen to children up to age four. Once your child has teeth, lift his or her lips once a month and check the teeth. Look for dull white spots or lines on the teeth. These may be on the necks of the teeth next to the gums. Dark teeth are also a sign of tooth decay.

If you see any signs, go to the dentist right away. Early childhood tooth decay must be treated quickly. If not, your child may have pain and infection.

If you give your child a bottle of milk, formula or juice at bedtime, stopping all at once will not be easy. Here are some tips:

  • Put plain water in the bottle.
  • If this is turned down, give your child a clean soother, a stuffed toy or a blanket.
  • If your child cries, do not give up.
  • Comfort him or her, and try again.
  • If this does not work, try watering down your child’s bottle over a week or two, until there is only plain water left.

Take a piece of floss and wrap it around your middle fingers, leaving about 2 inches between the hands. Use your index fingers to guide the floss between the teeth.

Slide the floss between the teeth and wrap it into a “C” shape. It should wrap around the base of the tooth, where the tooth meets the gum.

Step 3
Wipe the tooth from bottom to top 2 or 3 times or more, until it is clean.  Be sure you floss both sides of each tooth, and don’t forget the backs of the last molars.  Move to a new part of the floss as you move from tooth to tooth.

What are dentures?

Dentures are artificial replacements for your natural teeth and gums. If an accident, a disease or poor oral health care has left you with only a few healthy teeth or none at all, dentures can be fabricated to replace your missing teeth. Models of your teeth is made by taking impressions. The dentures are then custom made from these models.

Types of Dentures

Partial dentures are also called “removable partial prostheses” or “partials.” They may be used when nearby teeth are not strong enough to hold a bridge, or when more than just a few teeth are missing. Partial dentures are made up of one or more artificial teeth held in place by clasps that fit onto nearby natural teeth. You can take the partial out yourself, for cleaning and at night.

Complete dentures are what we most often refer to as “false teeth.” They are also called “full dentures” and are used when all your natural teeth are missing. Complete dentures are removable as they are held in place by suction. They can cause soreness at first and take some time to get used to. There are 2 types of complete dentures: immediate and conventional.

Immediate dentures are made before your teeth are removed. Your dentist takes measurements and makes models of your jaws during your first visit. Once your teeth are extracted, your dentist inserts the immediate dentures. The benefit of immediate dentures is that you are not without teeth during the healing period, which can take up to 6 months. During the healing period, your bones and gums can shrink and your immediate dentures may need to be relined by your dentist for a proper fit.

Conventional dentures are made and inserted into your mouth after your teeth have been extracted and the gums and jaw tissues have healed.

If you are having trouble with your complete dentures staying in place, overdentures can improve the retention of your dentures and allow you to speak and eat more effectively. An overdenture is a removable denture that fits over and attaches onto dental implants. The the implants stabilizes your overdenture and can improve the your comfort and decrease your chance of getting sore spots.

Caring for your Dentures

Complete and partial dentures need to be cleaned every day just like natural teeth. Otherwise, plaque and tartar can build up on your dentures and cause stains, bad breath and gum problems. Plaque from your dentures can also spread to your natural teeth and gums, causing gum disease and cavities.

To clean them, remove them from your mouth and run them under water to rinse off any loose food particles. Then wet a denture brush or a regular soft-bristle toothbrush and apply denture cleaner or a mild soap. Household cleaners are too abrasive and should not be used for cleaning dentures. Gently brush all surfaces of the prostheses including under the clasps where bacteria collect. Be careful not to damage the plastic or bend the attachments. Rinse your dentures well in clean water before placing them back in your mouth.

While your prostheses are removed, be sure to clean and massage your gums. If your toothbrush hurts your gums, run it under warm water to make it softer or try using a finger wrapped in a clean, damp cloth. If you have partial dentures, brush your natural teeth with a soft-bristled toothbrush and floss.

Always remove your prostheses overnight to give your mouth a chance to rest. Soak them in warm water with or without denture cleanser. If your dentures have metal clasps, only use warm water for soaking, as other soaking solutions can tarnish the metal. When you’re not wearing your dentures, keep them in water to stop them from drying out or warping. Never use hot water for soaking.

Dentures can break if dropped or squeezed too tightly. When you are handling your dentures, stand over a folded towel or a sink of water just in case you accidentally drop them.

Look for cracks in your prostheses. If you find any, you can take them to Capital Smiledocs for repair. If you have dentures it is important to see us regularly so that we can make sure that you are maintaining your dentures properly. Your mouth is always changing, so your prostheses will need adjusting or relining from time to time to make sure they fit well. Poorly fitted dentures may cause denture sores that make oral cancer more difficult to spot. At your dental exam, Capital Smiledocs will also examine your gums for any signs of disease or oral cancer and any natural teeth you may have for signs of decay or infection.

Bridges

A bridge, also called a “fixed bridge” or a “fixed dental prosthesis,” is a dental restoration that replaces one or more missing teeth. It extends across an area that has no teeth and is typically made up of an artificial tooth fused between 2 crowns. (A crown is a hollow, artificial tooth that fits over a natural tooth or a dental implant). The bridge is held firmly in place by your own teeth on each side of the missing one(s) or by dental implants. A bridge is permanent and cannot be removed.

If you have strong teeth on each side of a missing tooth (or teeth), these teeth can be used to support a bridge. If these teeth are not strong enough to support a bridge, then dental implants may be placed. A dental implant is an artificial root made of titanium that is inserted into the jawbone to replace the root of the natural tooth. The implant acts as an anchor to hold an artificial tooth or bridge in place.

How are bridges made?

Impressions (molds) are taken to create a model of your teeth. The model is then used to custom-make the bridge. While the bridge is being made by a laboratory, a temporary bridge is placed in your mouth to protect the teeth and gums.

During your second visit, the temporary bridge is removed and the custom made bridge bonded in your mouth.

Traditional bridges are used if there are natural teeth on each side of the gap where the tooth is missing.

Implant bridges are used if you don’t have healthy teeth or enough teeth to support a bridge, or when several or all teeth are missing. A custom-made bridge is anchored to the dental implants. Implant bridges can be used when all your teeth are missing.

If you take good care of your bridge, it should generally last for years. Like natural teeth, bridges need to be brushed and flossed every day. Are dental implants right for you?

Implants are not for everyone. You must be in good general health, have healthy gums and enough bone in your jaw to support the implant(s). You must be willing to see us several times until the work is done and take very good care of your implant(s). Must also see us for regular dental cleaning to help maintain your dental implants.

It’s normal for bone to shrink if it no longer has teeth to support. Because an implant sticks to bone, the bone is less likely to shrink. But if you have been missing teeth for some time, you may have lost bone. A bone graft can build up the bone so it can support an implant. When a bone graft is done, bone is added to the area where your jawbone has shrunk. Often at the time of the extraction of your teeth, a bone graft is needed to create more bone to hold the implants.

 

Here’s how an implant is done:

We will fabricate either crowns, bridges or dentures to anchor onto the posts, depending on your particular situation and needs.

Because implants stick to the bone, false teeth attached to implants look and act much like natural teeth. But implants are not as strong as natural teeth. You must brush and floss your implant(s) very carefully. Be gentle, but make sure you brush all sides of your implant(s). A toothbrush that has longer bristles at the tip may help clean behind your implant(s).

Floss very carefully at least once a day. You will need to be gentle with the floss where the implant meets the gum (called the gumline). You may find a floss threader useful for cleaning this area.

Braces or other appliances straighten your teeth so they look and function more efficiently. Who may need orthodontics? The position of your teeth and jaws has an effect on your bite. Your bite is how your top and bottom teeth come together. When your top and bottom teeth do not fit together properly, this is called a malocclusion or a bad bite. Problems like missing, crooked, crowded or protruding teeth can contribute to a “bad bite”. Thumb or finger sucking may also affect your bite.

An improper bite can make it hard to chew some foods and may cause some teeth to wear down. It can also cause muscle tension and pain. Teeth that stick out are more easily chipped or broken. Small brackets (or bands) are attached to each tooth with bonding. Arch wires are fitted into all the brackets and are usually held in place with rubber or wire ties. Braces put gentle pressure on your teeth. Over time, this pressure can move your teeth into the right position. Proper brushing and flossing is even more important for people with braces because plaque can easily build up around the edges of your braces. If this plaque is not properly removed, white or brown marks may appear on the enamel surface of your teeth and these marks may be permanent.

How do I care for my braces?
If you have braces, do not bite on hard things such as ice cubes and nuts. Do not chew the ends of pens or pencils. Also avoid sticky treats like chewing gum or toffee because they can loosen braces, distort wires and even pull braces off your teeth.

For mild malocclusions, Invisalign Clear Braces are another treatment option. They are clear, BPA-free acrylic aligners fit over teeth and work by exerting gentle pressure. This pressure incrementally moves teeth in the desired direction for a beautiful, healthy smile. The aligners are removable, which is an attractive feature. Oral Cancer is on the rise.

How many little sores or discoloured patches have you noticed in your mouth in the past few years? Most of the time you ignore them, and they simply go away. However, sometimes those lesions could be pre-cancerous or even cancer. What can make oral cancer hard to detect is most of the time they are painless. Some pre-cancer cannot be seen under normal white light and is only seen under fluorescent lighting.
Over the last four decades, the Human Papilloma Virus (HPV), known to many for its role in cervical cancer, has been showing up in increasing numbers of oral cancer cases. Oral cancer’s five-year survival rate is 80% to 85%. If found late, oral cancer’s five-year survival rate is only about 30%.
The VELscope system helps dentists visualize abnormalities in the mucosal tissues of your lips, mouth and upper throat. The examination is done in less than five minutes, without involving rinses, stains or discomfort. The VELscope’s blue light “excites” molecules deep within the layers of oral tissues. In turn, those moldecules emit their own light (fluorescence), in shades of green, yellow and red.
A recent 620-patient study at the University of Washington found that 69 (11.1%) of the routine dental patients examined had lesions only detected via VELscope assessment.

Did you know that veneers and crowns  won’t prevent you from getting cavities? A cavity is simply a hole in a tooth caused by decay that begins with plaque. Plaque is a sticky, yellow substance which contains bacteria that feed on the sugars left on our teeth by the foods we eat. These bacteria produce acid, which can eat through the enamel of the tooth, eventually causing a cavity.

Although harmful bacteria can’t penetrate porcelain veneers or crowns, they can infiltrate the areas of the tooth not covered by the veneer or crown.  The veneers and crowns are constructed from a mold of your teeth, and specially designed to cover dental flaws like chips and fractures, gaps or misalignment. However, the porcelain veneer or crown are only placed on the exterior of your natural teeth.  Therefore, the bottom root portion of your teeth are still vulnerable to cavities.

 The first sign of a toothache may be a dull ache, a clear indication the infection has spread from the enamel to the tooth pulp.  Biting down or chewing can be painful.  Left untreated, the tooth may break and need a root canal. At this point, treatment is more invasive.  To avoid this, take good care of your teeth by coming in for regular hygiene cleaning, even when you have porcelain veneers or crowns.
